New Patient Registration

bp_readingWe welcome and register any person living within the practice area. Your registration is with the practice and you are able to request to see any of the doctors at the practice.

Registration forms can be completed on line* or obtained from the reception desk.

Before you can be registered you must come into the Surgery with two forms of identification, one of which must be photographic ID, i.e passport or driving licence and the second proof of your permanent address i.e. a recent utility bill or bank statement.

*Please note, if you have not brought the above information into surgery within two weeks of completing the on line form, your registration application will have expired.

The registration process can take approximately two weeks, one of the clinicians may invite you to come in for a check-up if necessary, these patients will be contacted by letter. If you have not received a letter within a fortnight you can assume all is well and that you are registered with the practice.
