New Patient Registration

We welcome and register any person living within the practice area. Your registration is with the practice and you are able to request to see any of the doctors at the practice.

Registration forms can be completed on line* or obtained from the reception desk.

Before you can be registered you must come into the Surgery with two forms of identification, one of which must be photographic ID, i.e passport or driving licence and the second proof of your permanent address i.e. a recent utility bill or bank statement.

*Please note, if you have not brought the above information into surgery within two weeks of completing the on line form, your registration application will have expired.

The registration process can take approximately two weeks, one of the clinicians may invite you to come in for a check-up if necessary, these patients will be contacted by letter. If you have not received a letter within a fortnight you can assume all is well and that you are registered with the practice.

Pre-registration with the practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. You will be asked to sign the form to confirm that the details are correct.

Please note this is a pre registration form only, you will still need to come into the Surgery with identification (See Registration).

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. Again when you come to the surgery you will be asked to sign this form to confirm that the details are correct.
