What is Healthy You?

HealthyYou is a remote monitoring service, supporting clinicians as part of a remote care team to better manage people with a long term condition. You’ll be asked to upload vital readings relevant to your condition, from home with a device. The care team watch for any trends that indicate signs of a health decline, and intervene when necessary with the appropriate care. Preventing the need for unplanned GP appointments or in more extreme cases, admission into hospital. Our aim is to keep you healthy at home and better support you in managing your condition.

What are the benefits?

  2. Early detection to signal signs of decline, preventing the need for escalation into hospital.
  3. The care team reach out to you when escalation is required, rather than you having to call the surgery.
  4. Submitted readings can provide data to support in any annual reviews for your condition.

Why do I need to be monitored?

We’ve reached out to offer you remote monitoring as we know managing a long term condition can be difficult and life changing, and we want to offer you as much support as we can. There is no pressure to take part in remote monitoring if you don’t want to.

What happens if I notice my long term condition worsening whilst being monitored?

Your care team will be proactive towards your care to try and prevent the need for escalation, however if you are experiencing an emergency, dial 999 immediately. Otherwise, you must contact your care team, using the ‘contact the team’ button in the app, if you become unwell. Please do not wait for your care team to contact you.

How will my care team monitor my data?

Your care team may communicate with you via phone to discuss your readings and any necessary changes to your care plan.

How will my privacy and security be protected? 

Remote monitoring devices and platforms are designed with strict security and privacy protocols to protect patient data, and your care team will take steps to ensure that your information is kept secure and confidential.

How will my care team communicate with me about my readings? 

Your care team will review the readings you have inputted regularly, to track your progress and look out for any signs of your condition worsening.

How long will I need to use the remote monitoring device? 

The duration of remote monitoring will vary based on your individual health needs and treatment plan, and will be determined by your care team.
