Medical Exemption Certificate (FP92A)

A medical exemption certificate (FP92A) entitles you to free NHS prescriptions. 

Find out if you're entitled to NHS dental treatment and other help with health cost using the NHS eligibility checker.

If you do not qualify for the help that you need, you can apply for help.

You could be eligible depending on your household income and outgoings.

Please attend the medical centre and ask one of our receptionists for a medical exemption (FP92A) form.

Unfortunately electronic medical exemption forms are not available, they are availalbe on paper only.

When you have completed your form, please return to our reception front desk and we will arrange for it to be reviewed by a GP. If the clinician is in agreement with the information you have provided, this will be signed and sent off on your behalf.

Maternity Exemption Certificate

A maternity exemption certificate entitles you to free NHS prescriptions.

You can also use the certificate as proof that you are entitled to free NHS dental treatment while you are pregnant.

Speak to your midwife or health visitor. They will complete the application for you. They can do this as soon as they confirm that you’re pregnant.

If they use the digital maternity exemption service you can get your certificate by email as soon as they have completed the application.

Otherwise, you'll get a paper certificate in the post within 10 working days of us receiving your application.
